Thank you for choosing to use the services at our hospital. Patients are often unsure what is required of them before an admission, so we have set out the process step by step — and you can now complete the whole thing online.
The 48-hour head start
Before you are admitted to Johannesburg Surgical Hospital for a surgical procedure, there is a process to follow — the pre-admission. To make sure everything goes smoothly on the day of your procedure, we recommend completing it at least 48 hours beforehand. That gives us time to get you comfortable in your ward and properly prepped for surgery, instead of working through forms.
As part of the process you will complete a pre-admission form for the hospital. Once the three sources below have given you what you need, the form takes only a few minutes.
Doing it online is the quickest route — and if you have been admitted here before, we already hold most of it, so we only ask what has changed.

What you need, and who gives it to you
To complete the pre-admission successfully, the following information is required — and it comes from three places.
Your doctor
When your doctor decides to admit you for a procedure, they will provide you with the following medical codes:
- The diagnosis code (ICD-10)
- The procedure code (RPL / CPT)
- The practice number of your specialist (PCN)
Your medical aid
For the procedure to go ahead you need an authorisation number from your medical aid. You obtain it by giving them the medical codes from your doctor. Be sure you know exactly what your plan does — and does not — cover.
If you are not a member of a medical aid, a pre-calculated estimate will be provided to you for payment on or before admission.
You, the patient
Once you have the authorisation number from your medical aid, add it to the pre-admission form. You will also need to attach copies of the patient's:
- ID, passport or birth certificate
- Medical aid card
Do it online
The whole pre-admission from your phone, in about ten minutes. Sign in with your mobile number — there is no account to create and no password to remember.
- Photograph your ID and medical aid card; we check the photo is readable while the card is still in your hand
- We read the details off the card so you do not have to type them, and you confirm what we read
- Add your authorisation number when your medical aid issues it
- Come back later and we only ask what has changed — you never fill it in twice
Or on paper
If you would rather not do it on a phone, our admissions team will send you the printed A4 form. Complete it and bring it with your supporting documents.
Either way you will need the same three things — your doctor's codes, your medical aid authorisation number, and copies of your ID and medical aid card.

No authorisation number, no procedure
Medical aids will not fund a procedure without an authorisation number issued in advance. If you are struggling to obtain one, call our admissions team on 011 593 4483 before your procedure date rather than on the day.
Ready when you are
Complete your pre-admission at least 48 hours before your procedure. Our admissions and accounts teams would rather answer a question early than solve a problem on the day.
